GENERAL
Position Overview
Provides oversight to ensure the delivery of varied social work services to Hospice patients and their families. To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and community education, outreach and referral. Provides supervision and consults with the BSW on assessing the needs of patients and families, developing and updating the social work portion of the plan of care and delivering care to patients and families. supervision may occur in person, over the telephone, through electronic communication or any combination thereof. The Hospice social worker is an integral part of the Hospice Interdisciplinary Group (IDG).
MINIMUM QUALIFICATIONS
Education
Master’s degree in Social work from an accredited school.
Experience
One (1) year of experience in a health care setting.
Certification
Must be licensed by the Texas State Board of Social Work Examiners.
Knowledge Of
Basic computer skills; excellent interpersonal skills; able to communicate effectively in English, both verbally and in writing. Additional languages preferred.
Personality / Interpersonal Skills
Appreciation of and the desire to be part of a health care team dedicated to providing patients optimal care.
